*Please note: Cancellations outside 30 days will incur an administration fee of $100.00. Cancellations within 14 days will result in a 50% forfeit of registration fees. Cancellations/Unable to attend without notice will result in 100% forfeit of registration fees.

^#Accompanying persons are welcome for a registration fee of $300 which includes morning & afternoon teas, lunches and the conference dinner. Please submit this registration form word or pdf.
