Cyclone Larry Fruit Tree Losses Survey

The purpose of this survey is to determine the amount of damages sustained to industry & fruit tree crops by cyclone Larry . It is basically open to any fruit tree grower, with commercial or semi-commercial intent i.e partial income. By that it means, the couple of trees in your backyard wouldn't really qualify.

The survey also is acts as an indication of how many and what varieties of fruit trees are being grown in this area. You may distribute this to others who may be interested. It's open until around end of June.

You can fax it to DPI&F, South Johnstone at 4064 2249

Mail to: DPI&F, PO Box 20, South Johnstone, 4859

The other feature of this form is it's ideal as a basis to work out your claims. This form has cleverly broken down your plantings into 'chunks' allowing you to go through step by step.
